We are seeking a dedicated Care Partner / Case Manager to support clients in their own homes, ensuring they receive personalized, high-quality care under the new Support at Home program.

Key Responsibilities
- Conduct thorough initial and ongoing assessments to understand consumers’ needs, abilities, goals, preferences, and potential risks.
- Complete home visits for onboarding, review & assessment.
- Develop and regularly review person-centred care plans that reflect assessed funding levels and individual priorities.
- Prepare, monitor, and manage individual consumer budgets effectively.
- Ensure funds are used responsibly, supporting both immediate needs and long-term sustainability.
- Maintain precise, comprehensive, and compliant client records.
- Follow Aged Care Quality Standards, infection control protocols, and professional scope of practice requirements.
- Demonstrate a strong commitment to accountability and regulatory compliance in an evolving aged care environment.
What you will bring (must-have):
We are only considering candidates who can demonstrate:
- Current AHPRA registration as a Registered Nurse
- Proven experience as a Case Manager or Care Partner within Home Care setting
- Strong understanding of Home Care Packages, and consumer-directed care principles
- Excellent communication, problem-solving and documentation skills
- Passionate about supporting customers to live independently and achieve their goals
- Strong computer literacy and confidence adopting/using clinical and reporting systems.
- Commitment to privacy, confidentiality and respectful, person-centred care.
Appointment is subject to satisfactory Police Check, required vaccinations (including COVID), maintaining right to work in Australia, and maintaining AHPRA registration.
